Enrolment
Thank you for your interest in applying to enrol your child at Wembley Primary School. Please complete an Application for Enrolment Form by downloading the enrolment package from here or request from the School Office.
Proof of residence is required
When completing the Application for Enrolment Form you will also be required to provide evidence that you reside within the local intake area for Wembley Primary School. Applications cannot be considered until such time as documented evidence is provided to the school, (for Kindergarten applications we require the application form only until a placement has been confirmed).
Birth Certificate and Immunisation requirements
You will also be required to provide the student’s original Birth Certificate.
It is a requirement for you to provide your child’s current Australian Immunisation Register (AIR) Immunisation History Statement as part of the enrolment documentation.
The statement must be dated within 2 months of the date of application for enrolment. For more information, please click here.
Kindergarten Immunisation Requirements
On 22 July 2019, changes to the Public Health Act 2016 (WA) and the School Education Act 1999 (WA), came into effect that will better protect young and vulnerable WA children and the wider community from vaccine-preventable diseases.
Under these new changes, a child’s immunisation status must be ‘up-to-date’ according to their AIR Immunisation History Statement and must be dated within 2 months of the date of application for enrolment to be permitted to enrol into Kindergarten.
Pre-primary – Year 6 Immunisation Requirements
The AIR Immunisation History Statement must be dated within 2 months of the date of application for enrolment.
Children Vaccinated Overseas
If you move to Australia from overseas you need to make sure your child’s previous vaccinations are recorded on the Australian Immunisation Register (AIR).
When you arrive in Australia, take all documents about your child’s overseas vaccinations to an immunisation provider, such as a doctor. These documents should show:
- your child’s full name and date of birth
- name(s) of vaccines
- date of when each vaccine was given
- evidence that each vaccine was given (e.g. a stamp or nurse signature).

Overseas students
Students born overseas must hold either; an Australian passport, citizenship papers or appropriate visa before applying to the school. Students who do not hold an Australian passport during the application process must present their Visa Grant Form and passport at the time of enrolment.
